8D Methodology

D0–D3: Intake & Containment
The opening disciplines (D0 through D3) form the operational foundation of every 8D claim. They are designed to rapidly isolate risk, mobilize the right internal and external personnel, define the defect with indisputable technical clarity, and prevent non-conforming material from escaping to the customer.
D0: Intake, Champion & Opening Quality Gate
D0 establishes the metadata foundation and officially initiates the claim's lifecycle:
- Searchable Titles & Keyword Discipline: Enter a descriptive, keyword-rich title. The title is directly indexed for global search and historical case retrieval:
- Include Specific Keywords: Mention the physical defect type, component description, or production batch (e.g., "Surface Porosity on CNC Milled Housing Flange - Lot #4410").
- Avoid Generic Titles: Never reuse the same generic title across multiple claims (such as repeatedly using "Dimensional Defect" or "Customer Return"). Distinctive titles are essential to prevent search pollution and ensure instant retrieval during future audits.
- Target-Scoped Categories: Assign defect categories filtered by target (Customer or Supplier). These categories structure search indexing and feed into organization analytics to benchmark defect trends across your supply chain.
- Contextual Linking & Multi-Part Support:
- Link the affected partner Company (Customer or Supplier) and the relevant Project.
- Multi-Part Number Definitions: An 8D claim is not confined to a single item. Fast8D supports associating multiple different Part Numbers within a single claim, allowing you to specify individual affected quantities and distinct non-conformities for each component.
- Claim Initial Costs: Record the fixed financial costs associated directly with the claim that are already present right at the start of the analysis. These are not related to the resolution process or any operational tasks—they represent the upfront fixed impact, primarily customer-side liabilities such as process stop time / assembly line downtime charges, commercial refunds, debit notes, or initial scrapped material costs. Associating these fixed costs at D0 establishes the opening financial baseline of the claim before investigative and containment actions begin.
- Target-Scoped Claim Template: Assign a claim blueprint template tailored to the target scope. The template defines mandatory checklists, discipline deadlines, and quality gates.
- Champion Assignment: Designate the internal Champion responsible for steering the claim through to resolution.
- The First Quality Gate: Submitting D0 triggers the opening approval gate. This formal milestone officially logs claim opening and starts the response timer and SLA tracking clock. Other tenant members can be required as approvers; for solo workspace users, approving it immediately starts the lifecycle timers.
D1: Team Assembly & Stakeholder Roles
A successful 8D relies on cross-functional alignment. In Fast8D, teams are organized with clear responsibilities:
- Multidisciplinary Core Team: Assemble specialists across Quality, Engineering, Manufacturing, and Logistics.
- External Integration: Seamlessly invite external partners—such as the customer's logistics manager or the supplier's quality engineer—for direct collaboration within the claim without exposing unrelated tenant data.
- Active vs. Info Members:
- Active Members: Full participants to whom specific containment tasks, root-cause investigations, or corrective actions can be delegated.
- Info Members: Kept informed via automatic system notifications regarding claim opening, critical milestones, and final approvals without cluttering their task lists.
D2: Problem Description & Requirement Scoping
D2 is the single most critical foundation of the entire 8D investigation. An imprecise problem description leads to superficial root causes and failed corrective actions.
- Clarity on Requirement Deviation: Every failure mode must be crystal clear to all stakeholders. Identify the exact drawing dimension, functional specification, tolerance, or customer standard that each part is deviating from.
- 100% Failure Coverage: Every non-conforming part registered in the claim must have at least one associated failure description. Fast8D enforces complete failure coverage so that no affected component is left unscoped or unaddressed.

- Structuring Tools (5W2H & Metrology):
- While the 5W2H standard (Who, What, Where, When, Why, How, How Many) is a primary structuring framework, Fast8D encourages comprehensive documentation.
- Attach high-resolution defect photographs, CMM dimensional reports, boundary limit samples, and incoming inspection records to establish an objective factual baseline.
D3: Interim Containment Actions (ICA)
Containment actions exist to protect the customer immediately until permanent corrective actions are validated in D6.
In Fast8D, containment centers on three core requirements:
- 100% Failure Coverage: Every failure defined in the D2 problem description must be addressed by at least one containment action (quarantine, 100% sorting, rework, or production hold). No defect is permitted to remain uncontained.
- Direct Ownership: Every containment task must have a single designated owner directly responsible for execution.
- Strict Due Dates: Every action requires an explicit due date aligned with customer containment SLAs (e.g., 24h / 48h emergency protection windows) and records the completion date when done to guarantee prompt customer protection.
